Self-Pay Rates

• $350 per Initial Evaluation
• $200 per Follow Up Session

Insurance

I accept the following insurance:

• AETNA
• CIGNA
• United Health Care/UMR (with Headway Billing

I use HEADWAY BILLING for Aetna, Cigna, UHC/UMR. Client will be added to Headway before scheduling 1st appointment.

I’d recommend looking up this info before your initial appointment:

• What is my co-payment for medical Specialist? (often on card, often 2X PCP copayment)
• Do I have a deductible? If so, what is it and have I met it yet? If not met, the full charge will be due at date of service & reported to insurance.
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

Payment

I accept cash, check and Visa/Mastercard/AmEx and you are required to pay copayment on the date of service.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged a LATE CANCEL fee of $30. If you fail to show for an appointment, you will be charged $60 and this will be charged to your account on the date of the appointment.
